Medical Form
The Medical Form for kayitz (summer) 2010 must be completed by all staff and the parents/guardians of all campers, prior to arrival at camp. Print out the Medical Form, fill it in completely and send it to the camp office. Please check the camper and staff handbooks for further details on the medical form or contact the camp office if you have any questions.
